Just like a physical product app design starts by sketching out some ideas. But instead of designing the aesthetic the focus is on SYSTEMS DESIGN. What is the flow of the app, what choices does the user have at everystage. Essentially you map out the flow which ends up looking like a family tree. The home page at the top.

You can then start to map ou the design of the main pages. Just use post it notes on reactngles of paer to do this. Keep it basic and focused on functionality rather that the details and aesthetics of the design.

Once you have arough plan and sketched out some screens in paper you can get your user / stakeholders to test it. Below is a video of EE testing their initial app design. They have called this 'low fideltity' testing.
